Dental Assistant Requirements in Ohio: Registration, X-Ray Rules & Allowed Duties

Founder, DentistryHires
September 2026

Ohio does not license entry-level dental assistants; it regulates auxiliaries through built-up tiers under the licensed dentist's supervision.

The Administrative Code recognizes dental auxiliaries, basic qualified personnel, certified assistants, and expanded function dental auxiliaries, each authorized for progressively more advanced supportive tasks.

Expanded function dental auxiliaries and dental x-ray machine operators (dental assistant radiographers) must register with or hold a certificate from the State Dental Board; basic qualified personnel and certified assistants are not.

Radiographs require the separate x-ray machine operator certificate, generally under the dentist's direct supervision.

The tiers of dental assistant Ohio defines

πŸ“œ Rule 4715-11-01, 4715-11-02, 4715-11-03, 4715-11-04, 4715-12-01

Ohio's rules recognize dental auxiliaries generally, then build up through three specific tiers a licensed dentist may use under direct supervision: basic qualified personnel, certified assistants, and expanded function dental auxiliaries, each authorized for a broader set of intra-oral and extra-oral tasks than the last.

A separate chapter defines the dental x-ray machine operator, also called a dental assistant radiographer, a distinct certificate specifically for taking radiographs.

A licensed dentist may, in accordance with board rules, assign under the dentist's personal supervision and full responsibility, the concurrent performance of supportive procedures, to include assisting with the administration of drugs, medications, and inhalation anesthetic agents, including nitrous oxide-oxygen (N2O-O2) minimal sedation.

Registering as an expanded function dental auxiliary or dental x-ray machine operator

πŸ“œ Rule 4715-11-04.1, 4715-12-02

Expanded function dental auxiliaries must register with the board: a sworn application, a $20 fee, proof of a qualifying background (such as certification by the Dental Assisting National Board or the Ohio Commission on Dental Assistant Certification, dental student status, or dental hygiene licensure), completion of the required education, a passing exam score, and current basic life-support certification.

Dental x-ray machine operators instead apply for a certificate, pay a $32 fee, and complete a board-approved radiography training program unless an exemption applies.

Basic qualified personnel and certified assistants are not required to register with the board itself.

Each individual seeking to practice as an expanded function dental auxiliary shall register with the board in accordance with section 4715.62 of the Revised Code. An applicant for registration shall file with the secretary of the board a written application for registration, under oath, on a form the board shall prescribe and provide.

Personal supervision, direct supervision, and practice without the dentist present

πŸ“œ Rule 4715-11-01, 4715-11-03.2, 4715-11-04.4, 4715-12-01

These chapters use 'personal supervision' for a dentist assigning drug and medication assistance to dental auxiliaries, and 'direct supervision' for basic qualified personnel, certified assistants, and expanded function dental auxiliaries performing intra-oral and extra-oral tasks; neither term is defined here.

A certified assistant or expanded function dental auxiliary may treat a patient for up to fifteen consecutive business days without the dentist present if the assistant has two years and three thousand hours of experience, has completed a board-approved medical-emergency course, and the dentist has evaluated competence, examined the patient within the past year, and set written protocols.

A dental x-ray machine operator generally needs the dentist's direct supervision to take x-rays, unless the dentist examined the patient within the past year and ordered the procedures.

Except as provided in paragraph (C) of this rule, a dental x-ray machine operator may perform radiologic procedures only under the direct supervision of the supervising dentist. (C) A dental x-ray machine operator may perform radiologic procedures for a patient when the supervising dentist is not physically present at the location where the radiologic procedures are performed if the supervising dentist examined the patient not more than one year prior to the date the dental x-ray machine operator performs the radiologic procedures and the supervising dentist has ordered the radiologic procedures.

Tasks Ohio auxiliaries may perform, tier by tier

πŸ“œ Rule 4715-11-02, 4715-11-03, 4715-11-04

Under a dentist's direct supervision, basic qualified personnel may perform about forty listed basic remediable tasks, among them aspiration and retraction, instrument transfer, preliminary charting, several kinds of impressions, topical anesthetic and fluoride application, suture removal, placement of rubber dam over a preplaced clamp, provisional restoration placement and removal, orthodontic band and wire work, and vital-sign monitoring.

Certified assistants may additionally apply pit and fissure sealants and perform coronal polishing once they complete the specific training and certification each requires.

Expanded function dental auxiliaries may additionally place amalgam and non-metallic (including direct-bonded) restorative materials, on top of the pit and fissure sealants also open to certified assistants.

"Advanced remediable intra-oral dental tasks and/or procedures" - all tasks and/or procedures, with consideration of the soft tissue, involved in the art or placement of preventive or restorative materials limited to the following: (1) Pit and fissure sealants; (2) Amalgam restorative materials; and (3) Non-metallic restorative materials, including direct-bonded restorative materials.

Taking x-rays: the dental x-ray machine operator certificate

πŸ“œ Rule 4715-12-01, 4715-12-02

Only a certified dental x-ray machine operator, also called a dental assistant radiographer, may perform radiologic procedures, generally under the supervising dentist's direct supervision unless the dentist examined the patient within the past year and ordered the procedures.

To be certified, an applicant files an application, pays a $32 fee, and completes a board-approved dental radiography training program of at least seven hours plus clinical experience, unless already exempt for holding a Dental Assisting National Board, Ohio Commission on Dental Assistant Certification, or American Dental Technologists credential that included a radiography exam, an equivalent out-of-state license or certification, or qualifying work experience.

A dental x-ray machine operator may perform standard, diagnostic, radiologic procedures for the purpose of contributing to the provision of dental care to a dental patient.

Tasks no dental auxiliary may perform

πŸ“œ Rule 4715-11-06

The rule lists tasks that may never be delegated by a licensed dentist to any auxiliary: definitive diagnosis and treatment planning; final placement or removal of fixed or removable appliances and their therapeutic adjustment; cutting procedures on the tooth's coronal, root, or supportive structures; placement of the final root canal filling; final impressions for a prosthetic restoration (athletic mouthguard impressions are excluded); occlusal registration for a prosthetic restoration; final placement of prefabricated or cast restorations or crowns; and laser procedures other than caries-susceptibility testing devices.

The rule closes with a catch-all for anything else prohibited by law or the Administrative Code.

The following dental tasks and/or procedures shall not be delegated by any licensed dentists: (A) Definitive diagnosis and treatment planning. (B) The final placement of any fixed or removable appliances. (C) The final removal of any fixed appliance. (D) The therapeutic intra-oral adjustment of any fixed or removable appliance. (E) Cutting procedures utilized in the preparation of the coronal or root portion of the tooth. (F) Cutting procedures involving the supportive structures of the tooth. (G) The placement of the final root canal filling. (H) Final impressions of any tissue-bearing area, whether it be hard or soft tissue, upon which a prosthetic restoration is to be place. Taking of impressions for athletic mouthguards or similar appliances is not to be construed as a final impression. (I) Occlusal registration procedures for any prosthetic restoration, whether it be fixed or removable. (J) The final placement of prefabricated or cast restorations or crowns. (K) Procedures utilizing light amplification by stimulated emission of radiation (LASER) technologies (excluding caries susceptibility testing devices). (L) Any other dental tasks and/or procedures which are prohibited by law or agency 4715 of the Administrative Code.

Nitrous oxide (N2O-O2) monitoring by a dental assistant

πŸ“œ Rule 4715-11-02(B)(40), 4715-11-02.1

A dental assistant may monitor nitrous oxide-oxygen (N2O-O2) minimal sedation if at least eighteen years old, with two years and three thousand hours of dental-assisting experience, a current basic life-support certification, and a completed six-hour board-approved course covering the topics rule 4715-11-02.1 sets out.

The assistant may never administer, adjust, or terminate the sedation, may monitor only one patient at a time, must physically remain with the patient, and needs the supervising dentist's approval to discharge the patient.

Under no circumstances may the dental assistant administer, adjust, or terminate N2O-O2 minimal sedation. (f) The dental assistant shall not monitor more than one patient at a time. (g) The dental assistant shall physically remain with the patient at all times.

Continuing education for dental x-ray machine operators

πŸ“œ Rule 4715-12-03, 4715-12-05

A certified dental x-ray machine operator must register with the board by January first of each odd-numbered year, pay a $32 biennial registration fee, and show at least two hours of continuing education in dental radiation technology completed in the preceding two years; the certificate automatically expires if not renewed.

Continuing education topics may include radiation physics, biology, health and safety, film quality, radiographic technique, and digital imaging.

The chapters do not state a separate continuing education requirement for basic qualified personnel, certified assistants, or expanded function dental auxiliaries beyond the current basic life-support certification each must maintain or renew.

Every person certified to practice as a dental x-ray machine operator and required to register with the state dental board shall provide proof to the board at the time of applying for a renewal of registration that in the preceding two years the registrant has completed a minimum of two hours of continuing education in dental radiation technology.

Frequently Asked Questions

Do I need to register with the state to work as a dental assistant in Ohio?

It depends on the tier.

Basic qualified personnel and certified assistants are not required by these chapters to register with the State Dental Board itself, though a certified assistant's certification comes from the Dental Assisting National Board or the Ohio Commission on Dental Assistant Certification.

Expanded function dental auxiliaries must register with the board under rule 4715-11-04.1, and anyone taking x-rays needs a dental x-ray machine operator certificate under Chapter 4715-12.

Confirm your specific role with the Ohio State Dental Board.

Can a dental assistant take x-rays in Ohio?

Only as a certified dental x-ray machine operator, also called a dental assistant radiographer, under Chapter 4715-12 of the Administrative Code.

Getting certified takes an application, a $32 fee, and a board-approved radiography training program, unless the assistant already holds a qualifying Dental Assisting National Board, Ohio Commission on Dental Assistant Certification, or American Dental Technologists credential, an equivalent out-of-state credential, or qualifying work experience.

Certified operators generally need the dentist's direct supervision to take x-rays.

Confirm current exemptions and fees with the Ohio State Dental Board.

Does the dentist have to be in the office while a dental assistant works?

Usually a dentist must directly supervise basic qualified personnel, certified assistants, and expanded function dental auxiliaries.

But a certified assistant or expanded function dental auxiliary with at least two years and three thousand hours of experience, a completed medical-emergency course, and the dentist's documented protocols and recent patient exam may treat a patient for up to fifteen consecutive business days without the dentist physically present, per rule 4715-11-03.2 and 4715-11-04.4.

Verify the current rule with the Ohio State Dental Board before relying on it.

What can a dental assistant never do in Ohio?

Rule 4715-11-06 lists tasks no licensed dentist may delegate to any auxiliary, including definitive diagnosis and treatment planning, final placement or removal of appliances, cutting procedures on the tooth or supporting structures, placing a final root canal filling, most final impressions and occlusal registrations for prosthetic restorations, final placement of restorations or crowns, and most laser procedures.

The rule adds a catch-all for anything else prohibited by law or the Administrative Code.

Ask the Ohio State Dental Board before assuming an unlisted task is allowed.

How does a dental assistant become an expanded function dental auxiliary in Ohio?

The applicant registers with the state dental board: a sworn application, a $20 fee, proof of a qualifying background such as Dental Assisting National Board or Ohio Commission on Dental Assistant Certification certification, completion of a board-approved education program of at least one hundred eighty hours, a passing score on the required examination, and current basic life-support certification.

Registration must be renewed for two-year periods with a further $20 fee.
